BODY DONATION NORMS MEDICAL COLLEGE, KOLKATA

1. Collection of Pledge Form from the office of the Department of Anatomy of Medical College , Kolkata. (Ground Floor of the Anatomy Building).
2. Submission of duly filled in form at the same site with collection of Pledge Card/copy of the form (Receipt).
3. In case of natural death→ Relatives /next of kin of deceased has to bring the dead body in weekdays during working hour (9 am to 2 pm) along with the received Pledge form and Card. In case of death during holidays or beyond the working hours---dead body has to be preserved in hospital morgue temporarily (for which they have to contact the office of the MSVP), to be handed over the following working day.
4. Original Death Certificate and Photo ID proof of the Deceased and photo ID proof (preferably Aadhar Card) of the next of kin who will be accompanying and receiving the fresh Death Certificate cum Donation certificate from the department of Anatomy.

N.B:- Body of the Deceased must be accompanied by Original Death Certificate, duly filled in Pledge Form and Photo ID proof as mention above.
